* Provide healthy meals to families in our counties
* Partner with local grocery stores and food banks to link neighborhoods regular accesss to food
* Teach weekly virtual cooking classes and meal preparation
*Teach restricted diet education for individuals with conditions such as diabetes, end stage renal failure, high cholesterol and hypertension
SEXUAL HEALTH
*Virtual health coaching for chronic illness symptom management
*Connection to community resources for those with a STI diagnosis
*Facilitate engagement with healthcare provider for PEP/PrEP
*Sexual Health Screenings for high risk populations
*Provide rapid HIV and other STI testing in the community, with post education and support
*Provide transportation assistance- to and from medical appointments to increase engagement
COMMUNITY HEALTH
*Prescription drug cost coverage after insurance payment and/or up to a set amount
* 50% of initial visit costs for those establishing care at partnered clinics and medical facilities
*Co-pay coverage up to a set amount for dental and vision appointments
*Chronic illness education and medication management support
*Rapid testing, immunization, and vaccine education
*Arrange transportation - to and from medical appointments to increase engagement
*Mental and physical health screenings for the entire family based on evidence based practice and researched risk data
*Conduct community health screenings for children at risk for chronic illness and obesity
*Replacement of necessary equipment
*Harm reduction eduction and needle exchange resources
*Virtual coaching sessions for optimal health
